Position Overview:
The Social Worker plans, coordinates and manages social work services for frail older adults and caregivers of the Elder Service Plan (ESP) within the community as part of Integrated Disciplinary Team (IDT). Duties include conducting initial, semiannual, and annual assessments; participating in the Intake process for potential participants; providing one on one supportive counseling to ESP participants and their family members; and actively participates in the IDT Plan of Care meetings family meetings for IDT. Minimum Qualifications:
MSW from an accredited School of Social Work with current LICSW license, 1 year of experience working with an elderly population (paid or volunteer experience), 2 years of professional experience, of which 1 year was in providing direct behavioral health therapy to an adult population, Basic skills in Microsoft Office applications: Word, Excel and Outlook.

Required Certifications and Licenses:  Current MA LICSW license, 2 or more years of experience working with an elderly population and experience working with an interdisciplinary team preferred, Bilingual English/Spanish, English/Haitian Creole a plus.
Physical Demands of the Position:
Ability to work with elder clients of all diagnosis with end of life issues, ability to work in an open, hectic and noisy environment, ability to maneuver narrow hallways and climb stairs, Must be able to stand, sit, stoop, bend, walk, and reach, Must be able to lift up to 15 lbs., Must be able to push an adult of up to 300 lbs in a wheelchair, Must be able to articulate clearly, Must be able to hear faint and loud voices and noises, Must be able to read notes, prescription labels and other printed materials, Must be able to use senses of sight, smell and hearing to appropriately assess participants.
