Position Overview:
The Home Care Aide provides home health services to geriatric clients of the Elder Services Plan.  Under the general supervision of a professional, the C.N.A./Home Care Aide provides personal care at home, and care management as related to comfort, care and safety of a client.  Position will be based at our new facility at 479 Torrey Street in Brockton, MA. 
Minimum Qualifications:
 High School Diploma or GED, Competency and command of the English language (must be able to read, write and speak,) demonstrated ability to follow directions; directions maybe provided verbally or in written format, one year of experience working with a Geriatric population is required.
Required Certifications and Licenses: Home Health Aide Certification, Certified Nurse Assistant or a Personal Care Assistant Certification, one year of experience working with a Geriatric population is required, must be able to travel independently and timely to participant’s homes. 
Preferred Skills, Experience and Educational Requirements: 2 or more years experience with geriatric clients, preferably in a home setting, Course work in gerontology. 
Physical Demands of the Position:
Must be able to stand, sit, stoop, bend, walk, reach, climb stairs, and maneuver through narrow spaces or hallways, Must be able to assist clients with transfer from a lying position to sitting or standing position; from sitting position to standing or lying position; from standing to lying or sitting position using safe body mechanics, Must be able to assist clients with clothing (getting dressed) and with bathing activities using safe body mechanics, Must be able to lift up to 20 lbs., Must be able to push an adult of up to 300 lbs in a wheelchair, Must be able to articulate clearly, Must be able to hear faint and loud voices and noises, Must be able to read notes, prescription labels and other printed materials, Must be able to use visual sense to spot changes in physical condition, Follow directions and instructions both orally and in writing.
